Mission
We aim to improve access to quality medical, dental, and mental health care for low-income, uninsured men, women, and children in Utah County. We accomplish this goal by coordinating a Volunteer Provider Network, providing health education and maximizing existing community resources.
Purpose
Community Health Connect aims to improve healthcare access to low-income, uninsured, and minority populations in Utah County. Community Health Connect focuses on and coordinates efforts for more compelling, comprehensive, and efficient patient care by collaborating with social service agencies, healthcare practitioners, and various grassroots organizations.
History
Launched in July 1999, Community Health Connect is a 501(c)(3) supported by the Healthy Utah Valley group and the Roberts Wood Johnson Foundation. Community Health Connect is supported broadly by corporations, nonprofits, and regional foundations. The fiscal agent is United Way Community Services of Utah County, while Mountainlands Community Health Center handles administrative duties.
Program Components
Community Member Network
- CHC recruits, trains, and supervises people interested in volunteering to give back to their community.
- CHC volunteers provide medical translation services to physicians and assist new Medicaid or CHIP enrollees in finding approved providers with patient openings.
- CHC Care Coordinators serve consultants to provide community volunteers with training and guidance, assist clients, and work closely with social service agencies and the CHC providers to achieve patient care.
Volunteer Provider Network
- CHC organized a network of healthcare providers who donate their time and services to treat uninsured, low-income residents of Utah County.
Referral Coordination
- CHC accepts referrals from Mountainlands physicians, the Volunteer Care Clinic, and other medical centers in the area.
